To apply, you will just need information about:

Apply by Telephone: Must call 1-800-772-7213 from 7 a.m. to 7 p.m. Monday through Friday to schedule an appointment for a phone interview. The interview may take between one to two hours to complete. Those with a hearing impairment may call TTY at 1-800-325-0778 to request an appointment.
Submitting an SSDI Claim in Person: Claimants may call a local field office to request an appointment. Applicants may locate their local office by visiting the following tool found here on the Social Security Administration website.
